What should you ask at a diabetes follow-up appointment?

A short list makes the appointment considerably more useful: what my HbA1c is and what we are targeting; whether my current medication is still the right choice; when my kidney function and urine albumin were last checked; when my eyes were last examined, since retinopathy is asymptomatic until advanced; whether my feet have been examined; what my blood pressure and lipid targets are, since both matter as much as glucose in diabetes; and what I should do if readings go unusually high or low. Bring your readings and your full medication list including supplements.
